Access Center Navigator
A company is looking for an Access Center Navigator - Per Diem.
Key Responsibilities
Serve as a liaison for referring physicians, patients, and caregivers to facilitate appointment scheduling and service utilization
Navigate patients through the continuum of care and coordinate services, ensuring optimal transitions and addressing barriers to care
Perform data entry, maintain confidential records, and follow up with patients while collaborating with care teams
Required Qualifications
2-3 years of customer service or call center experience
High school diploma or equivalent; associate's degree preferred
Healthcare experience with knowledge of medical terminology and managed care preferred
Ability to use general office equipment, including a multi-line telephone system
Familiarity with databases such as Epic and CRM is beneficial
