You'll collaborate with interdisciplinary teams to identify trends, implement corrective actions, and elevate the overall standard of care for our residents.Key Responsibilities:Monitor and review Quality Measures and Five-Star Rating dataConduct regular audits of clinical documentation to ensure accuracy and complianceCollaborate with MDS Coordinators and interdisciplinary teams to improve documentation and resident outcomesEducate staff on best practices for improving quality metrics and regulatory complianceAnalyze trends and report findings to the Director of Nursing and facility leadershipAssist with developing and implementing performance improvement plans (PIPs)Participate in Quality Assurance & Performance Improvement (QAPI) meetings and initiatives Qualifications:Active RN license in the state of employment (Compact license accepted where applicable)Minimum of 2 years of experience in skilled nursing or long-term careExperience with MDS 3.0, CMS Quality Measures, and Five-Star Rating systemStrong clinical knowledge and attention to detailProficient in electronic health records (PointClickCare preferred)Excellent communication and training skillsAbility to collaborate across departments and lead quality improvement initiativesWhy Join us?We pride ourselves on delivering expert-led, compassionate care in an environment that values innovation, collaboration, and personal growth.
