Nurse Practitioner – Referral Partner&Home Care Assessments

We are looking for a Nurse Practitioner based in NYCwho has strong hospital or clinical access and a genuine desire to help patients transitionseamlessly into home-based care. This is a flexible, independent contractor role — perfect as a supplement to your current position. You refer patients who could benefit from our home care or Private Duty Nursing services, conduct assessments as needed, and earn competitive compensation for every successful referral and assessment. No minimum hours, no quotas — just meaningful work that pays.Your role• Refer qualified patients from your hospital, clinic, or community network to All HeartHomecare for home care or PDN services• Conduct PDN and general home care assessments for referred or agency-identified clients• Act as a bridge between the clinical environment and our care team, supporting smoothpatient transitions• Collaborate with our intake coordinators to help referred patients get enrolled quickly andappropriatelyIdeal background• Active NYS Nurse Practitioner license• Current or recent access to hospitals, discharge planning teams, or outpatient clinics in NYC• Familiarity with Private Duty Nursing (PDN), NHTD, TBI, or Medicaid home care programs is astrong advantage• Entrepreneurial mindset — motivated to build a referral relationship that benefits your patientsand your income• Professional, communicative, and collaborativeCompensation structurePerformance-based independent contractor compensation — earn per referral and per assessment,with no earnings cap.
