US Heart and Vascular is seeking a dedicated Nurse Practitioner to join our Value Base Care Integrated Care Clinic, a critical role in our multidisciplinary team focused on optimizing outpatient care for heart failure patients.Position Highlights:Full-time position in a collaborative clinic environmentMonday – FridayNo weekend/no callExcellent benefits packageResponsibilities & Duties:Under the supervision of a physician:Comprehensive management of patient panel and the patient conditions, including assessment, diagnosis, treatment, and consultation, on a longitudinal basisComprehensive documentation of patient chronic conditionsLeverage remote patient monitoring technology along with appropriate diagnostic tests to monitor patients' disease states and careGuideline directed medical therapy including prescribing, titration, and management for various cardiovascular disease statesAssess all patient and caregiver needs to establish goals of care and collaborates with interdisciplinary care team to manage patients care, including addressing social determinants of health, advanced care planning, transitions of care, and patient and caregiver educationAssesses patients' heart failure status and implement interventions to reduce acute care needs inclusive of addressing fluid volume overload in outpatient clinic settingAssess patients' cardiology disease state status and refer for health optimizing procedures such as ICD placement, ablation, and revascularization procedures as appropriateQualifications:Master's degree in nursing and completion of an accredited nurse practitioner programCertified Nurse Practitioner (NP) with an active AZ state license or willing to applyBLS and ACLS certificationsARNP Experience in a Heart Failure or Complex Cardiology Program or two (2) years of experience working in a cardiovascular setting is preferred but not requiredAbout Tucson, AZ:Tucson offers stunning desert landscapes, mountain views, and over 300 days of sunshine.
