Physical Therapist Location: Anaheim, CA Agency: Concentric Healthcare Staffing Pay: Competitive weekly pay (inquire for details) Shift Information: 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Job Title Physical Therapist (PT) - Home Health Location Mission Viejo, CA Schedule / Job Type Contract | Full-Time | Day Shift | Monday–Friday, 8:00 AM – 5:00 PM | Onsite / Field-Based Key Responsibilities Provide skilled physical therapy services to patients in a home health setting across an assigned territory Evaluate, develop, and implement individualized patient care plans in accordance with physician orders and clinical guidelines> Meet a weekly productivity standard of 30 points per week Document all patient care activities accurately and in a timely manner using HCHB (Homecare Homebase) EMR Participate in on-call and weekend coverage as required Coordinate care with interdisciplinary team members to ensure optimal patient outcomes Required Qualifications Active Physical Therapist (PT) license in the state of California Minimum of 1 year of home health experience with a Medicare-certified facility Proficiency with HCHB (Homecare Homebase) EMR system Ability to meet weekly productivity requirements Availability for on-call and weekend assignments Preferred Experience / Skills Familiarity with the service territory including Laguna Beach, Aliso Viejo, Dana Point, Laguna Hills, Laguna Woods, and Laguna Niguel areas Strong time management and self-directed work skills suited for a field-based role Why Join Us 13-week contract opportunity with guaranteed 40 hours per week providing stability and predictability Work in a rewarding home health environment making a direct impact on patient quality of life
