MDS Coordinator
MDS CoordinatorThe Glen is seeking a dedicated and detail-oriented MDS Coordinator to join our Nursing Leadership Team. This role is responsible for ensuring the timely and accurate completion of MDS assessments, Care Area Assessments (CAAs), and resident care plans in compliance with federal and state regulations. The MDS Coordinator plays a vital role in resident-centered care, quality outcomes, and accurate reimbursement.Why Join The Glen?Collaborative, resident-focused care environmentSupportive leadership and interdisciplinary teamworkOpportunity to impact quality outcomes and resident well-beingStability within a well-established organizationKey ResponsibilitiesEnsure timely and accurate completion of all MDS assessments, CAAs, and care plans in accordance with the current RAI Manual.Initiate and complete MDS assessments for non-Medicare residents, including resident interviews, hands-on assessments, medical record reviews, and staff interviews across all shifts.Coordinate interdisciplinary input from nursing, dietary, social services, therapy, and activities.Develop and update baseline and comprehensive care plans based on resident needs, risks, preferences, and advanced directives.Schedule, lead, and follow up on care plan conferences; address resident and family concerns.Track assessment due dates and coordinate with therapy and Medicare staff to ensure compliance and timely screenings.Ensure all required supporting documentation is present to support MDS coding, quality measures, and reimbursement.Collaborate with the Director of Nursing and Administrator on Quality Measures, Quality Indicators, Case Mix Index, and performance improvement initiatives.Participate in discharge planning, interdisciplinary rounds, and Quality Assurance/Performance Improvement (QAPI) activities.Communicate changes in resident condition to physicians and coordinate plans of care with the Medical Director.Assist with regulatory surveys, audits, and investigations.Serve as backup for facility submissions and assist Medicare staff during high census periods.Provide clinical support, staff guidance, and education as needed.Function as a household nurse leader or assist in clinical emergencies when required.RN MDS Coordinators will serve as the RN Assessment Coordinator.
