PRN Visit Registered Nurse (VRN) – Hospice CareLocation: Hattiesburg, MSStatus: Hourly | Non-ExemptAbout UsLegacy Hospice is seeking a compassionate and experienced Visit Registered Nurse (VRN) to join our dedicated team. In this role, you’ll provide vital care and support to patients in their homes, supplementing our RN Case Managers and ensuring timely responses to urgent needs.What You’ll DoPerform focused nursing assessments and address acute needs of hospice patients as directedEnsure continuity of care through effective communication with the interdisciplinary teamComplete initial and comprehensive assessments of patients and families to determine hospice needsDevelop and revise individualized care plans in collaboration with the patient and caregiverAdminister prescribed medications and treatments, and educate families on care techniquesCounsel patients and families on symptom management and end-of-life issuesMaintain accurate and timely clinical documentationAssist patients with activities of daily living and promote comfort-focused careRespond promptly to changes in patient condition or caregiver concernsCollaborate with case managers to ensure needed supplies and services are providedParticipate in interdisciplinary team meetings and quality improvement initiativesQualificationsRegistered Nurse (RN) licensed in the state of employmentCertified Hospice and Palliative Nurse (CHPN) and/or BSN preferredMinimum 1 year of hospice nursing experience required (2+ years preferred)Active Basic Life Support (BLS) certificationStrong written and verbal communication skillsSelf-motivated and capable of working with minimal supervisionValid driver's license, reliable vehicle, and proof of insurancePlease note: Legacy Hospice is a drug-free workplace.
