Direct Care ProfessionalTo assist consumers in performing the functions of daily living with as much independence as possible. Under general supervision, to guide consumers to be self-directed by teaching consumers decision making skills and encouraging independence as defined in each person's Personal Support/Habilitation Plan. To provide assistance in a manner that supports the personal dignity of the consumers.Shall comply with all rules, regulations, Policies and Procedures of the CompanyShall provide physical assistance to individuals as neededShall provide residential training to facilitate the acquisition of communication skills, sensorimotor skills, and daily living and self-help skillsShall provide assistance with daily living, such as ambulation, dressing, grooming, eating, toileting, bathing, meal planning and preparation, laundry, budgeting and financial assistance, home care and cleaningShall provide emotional support, advice and assistance to help the individual work through problemsShall communicate with the assigned Case Manager on a regularly scheduled basis and as neededShall provide or make arrangements for transportation to services, activities and medical appointments as neededShall actively participate in medical appointments and follow up care as directed by the medical staffShall cooperate with monitoring to include periodic announced and unannounced visits to the home by various monitoring entitiesShall assist individuals with the administration of their medications and ensure that medications are properly accounted for and properly labeledShall administer all aspects of residence operation; including development and maintenance of a household budgetShall assist with menu planning and grocery shopping as neededShall participate in household cleaning if necessaryProvide direct support to consumersAssist consumers in performing daily living functions such as personal hygiene, dining skills, household chores and community integration, etc.Ensures all personal goals are worked on a daily basis and appropriate documentation is completedAdministers medications at the appropriate time (S); notifies management and completes Medication Error form if an error occursAssist with meal service; teach dining skills and promote social skills and personal dignity during mealsAttend and successfully complete in-service training sessions as scheduledEnsures safe and clean living conditions at all timesComplete data collection, observation notes and daily reports as instructed.Ensures that individual has choice of community activities and provides safe transportationEnsures / provides safe transport of consumer (in personal vehicle) to and from destination(S) as needed while on dutyAssess the development of the consumer's abilities and interests to provide information for their Personal Support/Habilitation Plan.Participate in Interdisciplinary Team meetings to formulate the consumer's Personal Support/Habilitation Plans.Monitor consumer's activities to assure that safety and health care practices are observed.Ensure Universal precaution practices are observed.Reports to the Field Coordinator all incidentsMaintains confidentiality at all times and ensure HIPPA laws are followedEnsures all individuals retain cash receipt for all purchases; completes all appropriate financial documentationMonitors, addresses per individual's plan documents each individual's behaviors (S)Documents incident or injury on appropriate forms; notifies supervisor immediately in event of the injuryMust maintain all compliance requirements outlined for the position of Direct Care Staff; Reliable insured transportation, valid drivers licenses, current PPD (Mantoux TB test) and CPR and First AidEnsures / assists in follow-through re: special diet, therapies, treatments and medications as prescribedPerform related duties as required or assigned to fulfill the philosophy, goals and objective of the program siteFollows companies attendance policy and guidelinesFollow all policies and procedures as set fourth by Residential Living ServicesTreat consumers with dignity and respect.Promote independence, choice and empowerment of consumersProvides quality care to each individual according to his/her needsReports to management any activities, which may jeopardize individuals health and safety well being or any occurrences that go beyond Residential Living Services rules of conductShall plan and institute activities of choice for individuals living in the residenceShall ensure that staff notes are legible and completeShall ensure that individual training objectives are documented to ensure the individual is meeting or working toward personal goalsShall conduct randomly scheduled fire and evacuation drills in the home and document appropriatelyShall perform other duties as assigned by Field Coordinator or Operations TeamMust have a High School Diploma or GED and be at least eighteen (18) years of ageMust be a U.S. Citizen with a valid social security number or have appropriate work permit documentationMust be proficient in the monitoring and completion of various reports and recordsMust be proficient in handling crisis situationsMust pass a Criminal Record CheckMust have valid driver's license and be able to show proof of liability auto insurance (if applicable)
