If you're passionate about providing high-quality care to complex and underserved patient populations - and you value innovation, autonomy, and community-focused medicine - we'd love to meet you.About the RoleIn this unique and rewarding position, you will split your time between two vital programs serving the Wilmington community, with opportunities to practice women's health, pediatric care, and substance use disorder treatment in both settings.Mobile Health ServicesOur Mobile Health Services van brings comprehensive primary care - and elements of street medicine - directly to individuals and families throughout Wilmington. You'll deliver a broad range of services including:Wellness visits, sports physicals, and pediatric primary careWomen's health services (including preventive and reproductive health care)Chronic disease managementWound care, vaccinations, and sick visitsSupport for patients navigating substance use disorders, including medication-assisted treatment pathways (ie buprenorphine, vivitriol), harm-reduction education, and continuity of care connectionsYou'll rotate through community center locations, providing both walk-in and scheduled visits - offering autonomy in your clinical decision-making and the opportunity to build meaningful, long-term relationships across the community.Medical Respite ProgramLocated at the New Castle County Hope Center, the Medical Respite Program provides a safe, supportive environment for individuals facing medical or behavioral health challenges, often compounded by social or housing instability.
