The qualified applicant for this position should hold a Bachelor's or Master's degree in social work or other human services degrees will be considered, and have knowledge of the case management process and needs of geriatric patients, including funding resources. Responsibilities include, but are not limited to:working with individual patients and families toward adjustment to center life and to crises of illness, disability and deathserving in an advocacy role for patients and their families in expressing, defining and resolving grievancescoordinating discharge planning functionsreporting to the Director of Social Work Services the current status of social services and present needs participate in developing patient care plans in cooperation with individuals from other disciplinesmaintaining confidentiality of professional informationWhy NHC?We offer a culture of recognition, empowerment, and fun.
