Full-Time Employed: 40 hours/weekMon - Fri8:30am - 5:30pmLocation: 40 E Mitchell Dr # 100, Phoenix, AZ 85012(Additional Language Differential Pay Available)NOW OFFERING AN ADDITIONAL $1.50/hr SMI DIFFERENTIALFull Benefits Package, Including 401KGenerous PTO/Sick Time (4+ weeks in year 1)Duties May IncludeProvide inpatient visit to BHR within 3 days of admission to offer support and linkage to the Outpatient Clinical Treatment Team.Participate in individualized service planning that includes the involvement of the BHR to the fullest extent possible.Engage family/significant other, advocate, and /or designated representative involvement throughout the discharge and treatment process.Develop and execute a written discharge plan (Care Transition ISP) consistent with the BHRs TERROS Service Plan no later than the third day following a recipient’s admission to an inpatient facility. The discharge plan will be developed in collaboration with the BHR/designated representative/guardian, the TERROS treating Medical Provider, hospital treating physician, assigned outpatient clinician, and family, as appropriate.Offer or coordinate TERROS psychiatric services as appropriate, within 7 days of hospital discharge, and/or as needed to ensure adequate medication supplies.The Discharge Planner will maintain contact with the BHR/ involved family to evaluate patient status and efficacy of the service plan for a 30 day period.
