They facilitate admissions/discharges, advocate for resident rights, and mediate between families and the interdisciplinary care team.Key ResponsibilitiesKey ResponsibilitiesPsychosocial Assessments & Care Plans: Conduct comprehensive assessments upon admission and develop individualized, 90-day, or annual care plans.Discharge Planning: Coordinate with insurance managers, families, and community resources to manage safe discharges.Resident Advocacy & Rights: Ensure compliance with federal/state regulations, promote resident dignity, and protect rights.Family/Resident Support: Offer crisis intervention, counseling, and education to help residents adjust to long-term care.Interdisciplinary Collaboration: Participate in daily rounds, care conferences, and team meetings to review clinical, emotional, and social needs.Skills, Knowledge and ExpertiseRequired QualificationsEducation: Bachelor's or Master's degree in Social Work (BSW/MSW) from an accredited program.Licensure: Valid state licensure (LSW, LCSW, or equivalent).Experience: Previous experience in a nursing home, hospital, or geriatric care is preferred.Skills: Strong communication, documentation, and crisis management skills.
