Registered Nurse (Secure Residential) Job Overview Location/Schedule: The position is located at Barbara Roberts House, a secure licensed residential treatment facility located in NE Portland, OR, but will also float to two other secure facilities within the Portland area as needed. The schedule for this position Monday through Thursday, 8:30 a.m. to 5:00 p.m. Position: Registered Nurse Program: Secure Residential *Position includes a site and shift differential totaling $5.00 per/hour in addition to the base rate of compensation! Program Description : Secure Residential Treatment Programs serve adults living with severe and persistent mental illness (SPMI) who have a history of institutionalizing in the state hospital system. They are transitional facilities with a goal of helping residents become more independent. Due to the high level of support for the residents, these programs have a higher level of staffing. Position Description : The Registered Nurse (RN) for Secure Residential coordinates the provision of treatment for the shift and implements medical interventions. The RN assumes responsibility for treatment decisions in keeping with RN licensure provisions and renders care prescribed by physicians and nurse practitioners, while supporting a Trauma Informed Care environment. With the high prevalence of trauma among individuals receiving mental health services, it is required the RN understand the effects of trauma on health, coping, and other aspects of the lives of those we serve. Essential Responsibilities: This position description is not intended to be an all-inclusive list of responsibilities or an all inclusive job description. Provide quality nursing care, including all medical interventions and appropriate documentation. Formulate Nursing Care Plans for all medical/physical conditions. Administer PRN medications based on presenting systems and proper assessment to determine the need for the PRN. Participate in planning, implementing, evaluating and documenting direct resident care activities based on sound psychiatric and Arbor/Faulkner place philosophy. Act as liaison with medical staff, pharmacy and laboratory services when appropriate to assure continuity of quality resident care. Place strong emphasis on community reintegration, utilizing a comprehensive rehabilitation program that includes therapy and skills-building groups, dual-diagnosis treatment, nutrition and exercise, a vocational training program and staff-escorted community outings. Administer psychotropic medications, interpret resident response to medication and take appropriate action based on current psycho-pharmacy standards. Collaborate and participate as a team member to establish medical treatment plans and resident care. Interact appropriately with residents, visitors and other community members to support the treatment process, and advise on resident rights to treatment, refusal of treatment and involuntary treatment procedures as provided by applicable regulations. Document and maintain electronic health record (EHR) and ancillary records in a manner that is compliant and accurate. Attend and actively participate in Nursing meetings and resident case conferences. Assist in crisis situations as appropriate, following emergency protocols/procedures and coordinating with the treatment team with interventions which support a trauma informed environment for residents and staff. Demonstrate responsibility for safety of residents, staff and property; is familiar with fire regulations and evacuation procedures. Adhere to mandatory abuse reporting laws and HIPAA requirements, follow compliance and respect and clients rights Call the Licensed Medical Practitioner (LMP) with laboratory results. Lead resident nursing groups. Coordinate with oncoming shift regarding resident concerns, building/program needs, and any issues relevant to ensure the smooth operation of the program. Responsible for administration of seclusion and restraint techniques as defined in non-violent physical intervention training, agency Policies and Procedures, as well as applicable Oregon Administrative Rules. Be a part of a rotation to remain working on-site for up to eight (8) additional hours in the event that the following shift has inadequate staffing. Complete annual employee training requirements on a timely basis as indicated in the licensing requirements for the facility as well as agency requirements. Attend seminars, training and other educational opportunities in order to develop professional skills and abilities. Perform other duties as assigned Qualifications Education: An active, unencumbered Oregon Registered Nurse License and CPR/First Aid Certification. Must demonstrate the competencies to become credentialed as a QMHA or QMHP as outlined in OAR 309-016-0005. QMHP: A Bachelors degree in Nursing degree from an accredited college or university. QMHA: An Associates Degree in Nursing from an accredited college and at least one year of mental health work experience. Experience: A minimum of two years of clinical experience providing inpatient or outpatient services is preferred. Specialized Knowledge, Skills and Abilities: Knowledge of the kinds of medical conditions most commonly encountered by people with a severe and persistent mental illness and an understanding of the physiological basis for the nursing care delivered Motivational Interviewing Ability to maintain a trauma informed care environment Operate Computer hardware and software at a level to effectively perform the job functions Enter and retrieve data. Other: Ability to commute to various Cascadia sites and community locations as needed. Ability to perform CPR, First Aid, and properly use AED equipment. Working Conditions Work will be performed in a clinical outpatient or residential Setting. Meal periods and breaks are subject to interruption. There is moderate to high risk of personal injury/illness due to violent behavior, exposure to disease, biohazards, noise and contaminants etc. Daily routines may be hectic and will likely require the Registered Nurse to perform various functions and, on occasion, perform the tasks of other employees as needed. Mental Demands: The RN will have a varied routine which will require the ability to address new and usual circumstances in which outcomes may negatively affect costs, employee morale or clients. There is a degree of unpredictability, disruptions, requiring flexible time management. The position will be responsible for generating revenue by providing a level of service that meets company or contract standards. Physical Demands: A typical workday may consist of sitting, utilizing computers and other office equipment. Work may include transportation out in the community, using agency vehicles in order to support resident access to the community and participation in the community.
