Job Description Licensed Practical Nurse (LPN) – Long-Term CareLocation: Bismarck, NDDuration: 8-Week AssignmentShift: Nights (7:00 PM – 7:00 AM)Schedule: 12-Hour Shifts | 36/48-Hour RotationPosition OverviewWe are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care setting. The ideal candidate will support residents' physical, emotional, and medical needs while collaborating with an interdisciplinary healthcare team to ensure exceptional patient outcomes and resident-centered care.Key ResponsibilitiesProvide direct nursing care to residents in accordance with individualized care plans.Administer medications, treatments, and therapeutic interventions safely and accurately.Monitor and assess resident health conditions, documenting findings and reporting changes promptly.Assist with admissions, transfers, and discharge procedures.Maintain accurate and timely nursing documentation.Collaborate with physicians, registered nurses, therapists, and other healthcare professionals to coordinate care.Support implementation and evaluation of resident care plans.Promote resident safety, dignity, comfort, and quality of life.Ensure compliance with facility policies, nursing standards, and state regulations.Participate in quality improvement initiatives, infection prevention practices, and staff meetings.Educate residents and family members regarding care plans, medications, and health management.Respond effectively to resident needs, emergencies, and changes in condition.Required QualificationsEducationGraduate of an accredited Practical Nursing program.Licensure & CertificationsActive North Dakota Licensed Practical Nurse (LPN) license required.Current CPR/BLS Certification preferred.ExperiencePrevious experience in Long-Term Care (LTC), Skilled Nursing Facility (SNF), or geriatric care preferred.Skills & CompetenciesStrong clinical assessment and nursing skills.Knowledge of medication administration and resident care practices.Excellent communication and interpersonal abilities.Strong organizational and documentation skills.Ability to work independently and collaboratively within a healthcare team.Commitment to delivering compassionate, resident-focused care.Ability to work night shifts and flexible scheduling requirements.Work EnvironmentLong-Term Care / Skilled Nursing SettingCollaborative interdisciplinary healthcare teamResident-focused care environmentOpportunity to make a meaningful impact on resident health and well-beingFor more details reach at kpatel@navitashealth.com
