If you're excited to shape the future of home-based care, here's what the role involves.Columbus regionResponsibilitiesThe Home Health Aide provides personal care and related services, focusing on activities of daily living, in the home for the sick or disabled. As a Home Health Aide you will:Provide assistance with activities of daily living (i.e., personal hygiene) as part of the agreed-upon plan of careUnderstand and apply basic infection control principles and proceduresKeep a record of HHA Plan of Care for each patient assigned and provides updates to assigned case manager and patient's familyAttend required in-services and meetingsBe responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly CareQualificationsMust have and maintain an automobile to be used for work, current driver's license, good driving record and proof of insuranceA Home Care Aide certificateA minimum of one-year full-time experience in direct client care in an institutional setting (hospital or nursing facility)One-year full-time experience within the last five years in direct client care in an agency settingPerks and Benefits (What we offer):Competitive salary and multiple health benefit optionsCareer growth and developmentMentorship and continued learning opportunitiesEngaging and mission driven workplace401(k) matchingMileage reimbursement (if eligible)Every person on our team helps shape the future of Alpine Care Home Health.
