Registered Nurse

To provide nursing care in accordance with the patient's plan of care to includecomprehensive health and psychosocial evaluation, monitoring of the patient’scondition, health promotion and prevention coordination of services, teaching andtraining activities and direct nursing care.ResponsibilitiesCoordinates total patient care by conducting comprehensive health andpsychosocial evaluation, monitoring the patient's condition, promoting soundpreventive practices, coordinating services and teaching and training activitiesEvaluates the effectiveness of nursing service to the patient and family on anongoing basis Performs admission, transfer, re-certification, resumption of care anddischarge paperwork for the home care patient.Prepares and presents patient's record to the Clinical Record Reviewcommittee as indicated QualificationsCandidate must be a graduate of Bachelor’s Degree in NursingMust be licensed in Indiana as a Registered NurseA minimum of one year experience in community/home health agency or in a hospital setting is preferredMust be able to contribute to the quality of care being rendered through constructive communication with nursing managers and staffMust have a criminal background checkMust have a current CPR certification
