You'll support patients in maximizing independence and improving daily function through client-centered, evidence-based care.Compensation & Scheduling:Pay Range: $70.00 - $85.00 per routine visitStart of Care Visits: Up to $120 per SOCAvailability: 10-20 visits to start, with potential to grow into a full-time caseloadScheduling: Flexible hours designed to support work-life balance and your professional goalsFULL-TIME Earning Potential is $98,750 - $178,050PART-TIME Earning Potential is $40,250 - $107,330Key Responsibilities:Develop and implement individualized treatment plansConduct assessments using standardized toolsProvide hands-on, functional training for ADLs, mobility, and safetyEducate patients and caregivers on home programs and adaptive equipmentMaintain accurate and timely clinical documentationCollaborate with other clinicians as part of a coordinated care teamWhy Join Us:Make a real impact: Be part of a mission-driven organization where your work directly supports individuals and families receiving critical in-home care.
