This role is hands-on and patient-facing, providing direct clinical care, supporting the APRN, and serving as a clinical anchor for the team. The ideal candidate brings strong SNF experience, sound clinical judgment, and the ability to collaborate closely with providers, facility staff, patients, and families.Essential Duties & ResponsibilitiesKey ResponsibilitiesClinical CareRound daily on Align senior care patientsAssist APRN with clinical assessments and interventionsPerform Foley and suprapubic catheter changesComplete wound and dressing changesStart IVs and administer IV medications, including antibioticsAdminister IM medications (e.g., Lasix, antibiotics, prednisone, dexamethasone)Provide nebulizer treatmentsComplete timely and accurate nursing documentationCare Coordination & Team CollaborationAssist Care Ally with Health Risk AssessmentsSupport facilities with coordination of care and follow-upParticipate in clinical rounds and provide patient reports as neededEngage in family conferences when requestedCommunicate patient updates with families and facility staffQualificationsRequiredActive RN or LPN license in good standingStrong IV therapy and medication administration skillsExcellent documentation and communication skillsAbility to work independently and make sound clinical decisionsWillingness to work 10:00 AM - 7:00 PM and flexible scheduling, including occasional weekendsPreferredPrevious leadership experience (Unit Manager, Shift Supervisor, ADON, or similar)Skilled Nursing Facility (SNF) experienceExperience serving as a clinical lead or resource for staffMultilingual abilities based on facility needsFlexibility to rotate schedules (e.g., Mon-Fri one week, Tues-Sat the next)
