Job Description JOB SUMMARY: Professional member of the home health team who provides skilled nursing visits inpatient's homes under the direction of plan of treatment established with physician and input frompatient/caregiver.DUTIES AND RESPONSIBILITIES:Maintain and conduct self in a professional manner at all times and situationsMaintain strict confidentiality of all patient, employee and Haven Specialty Home Care's informationParticipate in all Haven Specialty Home Care's in-service programsParticipation in the nursing process by providing input to the client's plan of careImplementing the care plan as written6.Evaluating effectiveness of care plan and make recommendations for revisions as appropriateRecords all pertinent observations and treatments as per Haven Specialty Home Care's policies. Prepares clinical progress notes and provide appropriate documentation of service in keeping with Haven Specialty Home Care's policies and proceduresProvide for the emotional and physical comfort and safety of client taking into consideration their rights and cultural backgroundPerform nursing procedures for which he/she has been trainedNotify appropriate person of unusual symptoms and/or changes in client's conditionParticipate in case conferences, team meetings, staff meetings and performance improvement activities as assignedProvides proof of current license by state.
