**Job Summary and Responsibilities**We are seeking a skilled and compassionate **Occupational Therapist** to join CHI Franciscan Home Health team. The OT will provide skilled therapy services to patients in their home evaluating the patients abilities and needs, setting goals and developing a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems. **Now Hiring an Occupational Therapist for the Port Orchard area!****Responsibilities:**+ Assess patients' physical, cognitive, and psychosocial abilities to determine therapy needs.Develop and implement personalized treatment plans that may include exercises', adaptive techniques, and use of assistive devices to improve patients functional abilities.+ Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.+ Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.+ Monitor patients progress and adjust treatment plans as necessary to meet changing needs.
