If you have the clinical skill and the passion to make every session count, we’d love to talk to you.Key Responsibilities:Facilitate high-quality, person-centered TBS groups that promote emotional regulation, coping skills, social connection, motivation, and improved quality of life for nursing home residents.Adapt group content and activities to meet the diverse cognitive, emotional, and physical needs of older adults, including those with medical complexities, mild cognitive impairment, or other challenges.Build trusting, respectful relationships with residents while creating a safe, inclusive, and non-judgmental group environment that encourages participation and fosters a sense of belonging.Collaborate closely with nursing home staff, our on-site clinical team, and other care providers to ensure coordinated, consistent, and compassionate support for residents.Complete accurate and timely documentation, including group notes, progress updates, biopsychosocial assessments, and Medicaid-compliant treatment planning paperwork.Participate in regular clinical supervision, team meetings, and ongoing professional development opportunities within a supportive behavioral health team.Support overall resident engagement by identifying needs and helping integrate therapeutic gains into daily life.Qualifications:Must hold a current Ohio license as one of the following: LSW, LISW, LPC, or LPCC.Experience facilitating behavioral health groups (mental health or substance use) is required.Previous experience working with older adults, long-term care settings, or medically complex populations is strongly preferred.Strong interpersonal and communication skills with the ability to connect empathetically, patiently, and respectfully with residents.Ability to work independently while providing services in off-site nursing home facilities.Reliable transportation and a valid driver’s license are required (mileage reimbursement provided).Must be able to successfully complete Ohio Medicaid provider requirements and all necessary background checks.Preferred Skills & AttributesWarm, genuine, and compassionate approach with a true interest in supporting the dignity and well-being of aging adults.Familiarity with EHR systems (training provided if needed).Training or willingness to obtain training in Trauma-Informed Care within the first 90 days.Comfort working in nursing home environments, including the ability to walk/stand as needed during shifts.Flexibility and adaptability, with comfort traveling between partner facility locations as required.Compensation:Competitive Salary: $60,000 – $90,000 annually, based on licensure and experience.Work-Life Balance: Flexible daytime scheduling (primarily 9:00 AM – 5:00 PM) and mileage reimbursement.Travel Support: Mileage reimbursement provided for traveling between partner facility locations as required.Financial Incentives: Sign-on bonus for qualified immediate starts, plus ongoing referral bonuses.Generous Time Off: 18 days of PTO, 6 paid holidays, and a paid birthday holiday.Growth & Support: CEU support, professional development, and clear pathways to clinical leadership roles.
