About the RoleWe are a stealth-mode, venture-backed healthcare startup reimagining long-term cancer follow-up care. Our mission is to expand access to high-quality survivorship services beyond traditional academic centers through scalable, patient-centered, primarily virtual care models.This role is designed for a provider excited to deliver care in a telehealth-first environment, where all patient visits are conducted virtually. This structure enables flexibility while allowing for high-touch, longitudinal patient relationships at scale.We are seeking a Nurse Practitioner to lead and help build a national Cancer Survivorship Program. This is a high-impact role focused on shaping care delivery models, supporting clinical operations, and directly caring for patients.Key ResponsibilitiesConduct virtual survivorship visits and develop individualized, patient-centered care plansManage late and long-term effects of cancer treatment, including chronic condition managementCoordinate evidence-based surveillance and follow-up care for recurrence monitoringProvide patient education, counseling, and psychosocial support in a virtual settingHelp design and refine clinical workflows, SOPs, and care protocolsCollaborate with multidisciplinary teams to ensure coordinated patient careSupport care delivery optimization, quality improvement, and patient experience initiativesContribute to scaling the program nationally through operational and clinical inputQualificationsNurse Practitioner (Master’s or DNP) with active license and DEA (or eligibility)Strong clinical judgment and ability to deliver high-quality care in a telehealth-first modelExcellent communication skills with a warm, empathetic, and patient-centered approachComfort working in a fast-paced, evolving startup environmentAbility to quickly learn structured care pathways and apply them consistently in practicePrior Oncology & Survivorship Experience preferredSkills & AttributesStrong interpersonal communication and patient engagement skillsInterest in cancer survivorship care and chronic disease managementAbility to work independently while collaborating across teamsAdaptability and comfort with evolving clinical workflows and toolsStrong organizational skills and attention to detailNice to Have Familiarity with ASCO, NCCN, or other evidence-based survivorship guidelines (training provided)Bilingual or multilingual skills are a plus, as they enable us to better route and serve patients across diverse language needs
