MDS Coordinator

MDS CoordinatorResponsibilities:Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments.Ensure timely and accurate submission of MDS data to state and federal agencies.Work closely with interdisciplinary team (nursing, therapy, social services) to ensure that all aspects of resident care are accurately reflected on the MDS.Monitor resident status and assure assessments are completed in response to significant changes.Participate in the development of care plans based on MDS data.Ensure compliance with Medicare and Medicaid regulations.Requirements:Experience PreferredHave a current LPN or RN license in the State of OklahomaBe proficient in Microsoft Office Suites & PCCHave excellent interpersonal and communication skillsHave Skilled Nursing and Long Term Care experienceBe adaptable to change with a positive attitudeBenefits:MedicalDentalVisionOther Voluntary BenefitsRetirement PlanPTO planAbout our Health Center: Emerald Care Center Tulsa is in Tulsa, OK.
