Physician, Internal Medicine
Internal Medicine PhysicianThe Internal Medicine Physician role involves providing high-quality, comprehensive healthcare services to a diverse patient population within and outpatient setting. The physician will work in a collaborative environment with other healthcare providers to deliver accessible, patient-centered, and compassionate care. This role includes direct patient care, health education, and supporting the organization's community health initiatives and preventive care efforts.
Essential Duties and Responsibilities:
Provide direct clinical care to a diverse patient panel in an outpatient clinic setting, managing a wide range of health conditions including acute and chronic diseases.
Perform comprehensive health assessments, including physical exams, diagnostic tests, and the development of treatment plans.
Prescribe medications, administer immunizations, and monitor ongoing care for patients with acute and chronic health conditions.
Provide preventative care services, including health screenings, wellness checks, and chronic diseases management.
Collaborate with other healthcare providers, including specialists, nurse practitioners, medical assistants, and nurses to ensure comprehensive patient care and to coordinate referrals for specialized care as needed.
Deliver culturally sensitive care, considering social determinants of health, and provide services to underserved and vulnerable populations.
Participate in community outreach efforts, providing health education and promoting disease prevention and awareness within the local community.
Attend all relevant clinical department meetings, pertinent health care facility site meetings, and any additional meetings that may arise, necessitating the provider's participation.
Document all patient encounters accurately and timely in the Electronic Health Record (EHR).
Ensure compliance with healthcare policies, procedures, and regulations, including FQHC-specific guidelines, and advancements in internal medicine.
Act as a role model for professionalism and effective communication within the healthcare team.
Adhere to confidentiality guidelines regarding patient information and organizational protocols.
Comply with applicable laws and regulations related to collaborative agreements and licensure.
Comply with SIHF Healthcare policies and procedures.
Perform other duties as assigned.
Knowledge, Skills, Abilities:
Extensive knowledge in internal medicine, including the management of both acute and chronic conditions, disease prevention, and health promotion.
Strong ability to provide patient care in a diverse, multi-cultural, and underserved community.
Ability to work effectively within a multidisciplinary team of healthcare professionals in a collaborative, patient-centered environment.
Strong communication skills, with the ability to educate and engage patients in their care, providing clear and compassionate explanations of treatment options.
Proficiency in using Electronic Health Records (EHR) systems and other healthcare technologies.
Ability to manage a caseload of patients, balancing clinical duties with administrative responsibilities.
Cultural competency to deliver care to patients from various racial, ethnic, and socioeconomic backgrounds.
Expertise in preventive care, chronic disease management, and patient education.
Strong organizational and time management skills, ensuring efficient patient flow and quality care.
Ability to adapt to the demands and challenges of working in an FQHC setting, particularly in high-need, vulnerable patient population.
Strong critical thinking and problem-solving skills, capable of making informed clinical decisions in a fast-paced, dynamic environment.
Education and Experience:
Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.
Board certified or board-eligible in Internal Medicine.
Licensed to practice in the state of Illinois or eligibility for licensure in the state of Illinois.
Two plus years of experience as an Internal Medicine Physician preferred.
Physical Demands:
Sedentary physical activity performing non-strenuous daily activities of an administrative nature
Work Schedule:
Normal work hours for this position are Monday through Friday dayshift, unless otherwise specified by management staff. Travel to other clinical locations may be required.
