LVN / LPN - LTC/SNF
Job Description Job Summary: Provide skilled nursing care as an LPN in a long-term care or skilled nursing facility setting, ensuring patient safety and comfort. Deliver high-quality nursing services while adhering to regulatory standards and facility policies. Responsibilities: Administer medications and treatments accurately according to physician orders. Monitor patients' conditions and report changes to the healthcare team. Document patient assessments, interventions, and responses in accordance with protocol. Collaborate with interdisciplinary team members to develop and update care plans. Follow all safety and infection control procedures to maintain a safe environment. Manage and prioritize multiple patient needs during assigned shifts. Qualifications: Hold an active NYS LPN license with at least 2 years of LTC/SNF experience. Possess current CPR/BLS certification. Demonstrate strong clinical skills and knowledge of nursing procedures. Maintain flexibility to work within an assigned travel radius of 50 miles. Be able to work independently and as part of a team.
