Description: RESIDENT CARE COORDINATOR/LPN REQUIRED (Full time Needed from 11-am to 7-pm Sunday - Thursday) A current active license as a Practical Nurse in the State of Arizona is necessary. Who you are: The Resident Care Coordinator/LPN is responsible for the daily functioning of the health care needs of the residents in the community. This includes supervising the resident assistant, supervising the medication delivery system, training of new staff, and in-servicing needs of existing staff.What you will do: Reviews and documents daily in the communication log as appropriate. Reviews new orders and MAR daily. Supervises and reviews the transcription process, medication and equipment ordering, and pharmacy cycle fill. Performs resident care coordination and communication with outside agencies and medical providers. Reviews alert charting. Reviews incident reports as needed. Reviews cleanliness and organization of medication room and medication carts. Reviews, supervises, and assists with resident care as needed. Performs medication pass as needed. Assists residents with meals as needed. Reviews and documents the service plan as needed. Coordinate and attend service plan reviews. Reviews and responds to Community RN audit to ensure compliance. Assist and/or conduct the orientation for new employees. Schedule the exit-training interview with new employees. Performs all other duties as assigned by any member of management. Participates in every other week with staffing on call duties. Always takes necessary measures for adequate staffing. What you bring: Ability to utilize and apply basic management skills. Provides and coordinates resident care utilizing the principles of assisted living per the rules of the state where employed. Maintains adequate attendance record, notifies supervisor if late or unable to work in accordance with personnel policies. Requires minimal supervision, is a self-starter and flexible. Organizes and utilizes time appropriately, sets priorities and accomplishes assigned tasks. Displays mature behavior in attitude, speech, and action. Keeps resident information confidential, respects resident rights, respects privacy and the right to self-determination by the residents. Works cooperatively with supervisors and co-workers. Abides by the established policies and procedures of the Community. Looks for ways to improve Community function. Has the ability to spend long periods of time on your feet and ambulate quickly. Is accurate, dependable, neat, and thorough in assigned tasks. Can communicate effectively, verbally, and in writing. Other Requirements: Current active license as a Practical Nurse in the State of Arizona is necessary. Previous work with the elderly population is desirable. Previous work as a medication assistant is desirable. Maintains CPR Certification. Universal Precautions: Exposure to blood/bodily fluids is unlikely. Physical Requirements: Ability to lift 40-50 pounds regularly during work hours (heavy work level) Job Type: Full-time Work Location: In person
