Pediatrics Integrated Pharmacist - ED / PICU (Mid-Level)Location: Houston, TX 77030Full TimeShift: 7 On / 7 Off | Evening Shift (12:00 PM - 11:00 PM)Department Support: Emergency Department (ED) + Pediatric ICU (PICU)Certification Preferred: PALSResidency-Trained Integrated Pediatrics Pharmacist to join the team supporting the Pediatric Emergency Department (ED) and Pediatric Intensive Care Unit (PICU). This is a mid-level clinical pharmacy role on a 7 on / 7 off evening schedule (12 PM - 11 PM).The pharmacist will be responsible for safe and accurate medication dispensing, clinical interventions, medication profile reviews, IV admixture oversight, and collaborative multidisciplinary support to ensure optimal pediatric medication outcomes.Key ResponsibilitiesClinical & Patient Care SupportProvide drug-related information and education to patients, families, hospital staff, and providersPerform clinical interventions including:Medication profile reviewIV-to-oral conversionRenal dosing adjustmentsAntimicrobial streamliningTherapeutic drug monitoringVancomycin/aminoglycoside dosingMedication historiesPatient counselingParticipation in multidisciplinary roundsDocument all clinical interventions appropriatelyMedication Dispensing & IV SupportFill and label prescriptions for inpatient and outpatient needsMaintain medication profiles and identify drug-related problemsEnsure safe and timely preparation/scheduling of:IV solutionsAdditivesSterile compounded products (as applicable)Dispense controlled substances and maintain inventory documentation and complianceLeadership / Training / Department SupportSupervise pharmacy technical support staffMentor/train pharmacists, pharmacy students, and residentsAssume department management responsibilities when Pharmacy Manager/Lead is absentParticipate in:Medication Use Evaluation (MUE)Process improvement programsPolicy/procedure drafting and reviewNewsletter collaboration as neededSafety, Quality & ComplianceInitiate reports for medication errors/adverse interactionsFollow all Memorial Hermann standards, policies, and proceduresSupport productivity, quality outcomes, and supply/time management expectationsPromote continuous education and competency developmentMaintain a patient-centered approach aligned with Memorial Hermann service standardsRequired Qualifications (Must-Haves)Education: Graduate of an accredited School of PharmacyLicensureActive Texas Registered Pharmacist licenseSterile Product Certification (required if compounding sterile products)Through in-house or accredited programExperienceMinimum 2 years of hospital pharmacist experienceKnowledge of unit dose medication systems and IV admixture processesPreferred Qualifications (Nice-to-Haves)Residency training (strongly preferred for integrated pediatrics role)Pediatric integrated clinical experience supporting:EDPICUPALS Certification (preferred)Core Skills / KeywordsPediatric Clinical PharmacyED / Emergency Medicine SupportPICU / ICU PharmacistSterile Compounding / IV AdmixtureUnit Dose Medication ProcessesMedication Profile ReviewTDM (Therapeutic Drug Monitoring)Antimicrobial StewardshipVancomycin / Aminoglycoside DosingClinical Interventions DocumentationMultidisciplinary RoundsControlled Substance CompliancePharmacy Technician SupervisionQuality Improvement / MUERecruiter Submission Template -Full Name:Degree Major with University and Completion Year:Are you a Graduate of an accredited School of Pharmacy?
