Your recruiter will provide the most up-to-date details during the hiring process._**How you'll make an impact in this role**+ Lead multidisciplinary care as the primary coordinator for all clinical teams to ensure every patient receives a seamless and well-integrated treatment experience.+ Manage recovery journeys by implementing personalized care plans and monitoring clinical conditions to provide timely, life-saving interventions.+ Advocate for patient safety through the diligent execution of physician orders and the maintenance of high-quality health records.+ Empower patient transitions by collaborating with the healthcare team to create comprehensive discharge plans that ensure success beyond the hospital walls.**What minimum requirements you'll need**Licensure / Certification / Registration:+ BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
