RN/LPN 30-40hrs a week

Our mission is to provide the highest quality of care for our patient/clients by matching people with meaningful, long-lasting, and enjoyable careers, not just a job.Responsibilities:Take and record Vital signs including fluid intake and outputMonitor patient's condition and report changes/concerns to appropriate medical professionals and family membersProvide Activities of Daily Living care as outlined in the Care Plan-bathing, dressing, feeding, etc.Transfers patient with equipment using appropriate safety measures (gait belt, hoyer, sit-to-stand lifts)Perform light housekeeping duties to maintain a clean-living space for clients. Ability to turn and reposition patientsUnderstands First Aid and Emergency Care protocolsComplete electronic documentation while on shiftMedication remindersCatheter careAdminister prescribed medications including injections, enemas/suppositories, etc as ordered by PhysicianTrach and vent careFeeding tube careWound careSkills:Familiarity with Home Health Care, safety and sanitation standards and proceduresUnderstanding of Confidentiality and Privacy LawsExcellent communication and interpersonal skills… able to communicate effectively with patients and their families.Knowledge of basic medical terminologyDetailed orientedAbility to turn and reposition patientsAbility to undertake a variety of diverse care tasks and work as a team memberCompassionate and able to handle tough situations as needed1 year experience as a Nurse/CNA/Home Health AidAdequate knowledge of medical terminologySuccessful completion of a LPN or RN license program is a mustUnderstanding of Nursing Best PracticesTrach and vent experience is a MUSTCarePlus TLC is a locally owned small business with a desire to give back to our communities in impactful ways!
