Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active Board Certified in ABEM only Active FL License ACLS/BLS/PALS/ATLS Providers living inside of 50 miles from the facility will be considered "Local
