For frequently asked questions, please click here: https://www.catholiccharitiestrenton.org/careers/frequently-asked-questions/Req NumberDIR-25-00091LocationIBHS Burlington - EISS (Westampton, NJ)Full-Time/Part-TimePart-TimeNumber of Openings1CategoryDirect Care - Professionals / ParaprofessionalsDescriptionJOB SUMMARY: Provides peer support to individuals receiving mental health support services through the EISS program.ESSENTIAL FUNCTIONS:Provides on-site support services which include but are not limited to personal care assistance (hygiene, grooming), life skill development (nutrition, money management, medication monitoring, recreation and socialization), entitlement management and advocacy, and supportive counseling (emotional support, job coach/vocational support, substance abuse support).Participates in treatment planning, ongoing assessment and discharge review process.Responsible for maintaining accurate and current consumer records.Responsible for the development of Wellness Action Recovery (WRAP) Plans with program participants.Contributes to and participates in interdisciplinary in-service trainings and case conferences.Participates in on-going supervision and/or program development with the Program Supervisor, which may consist of field visits to consumer residences, work sites, families, etc.EISS specific responsibilities:Communicates observations and information about program participants to the program supervisor, RN and treatment team.SUPERVISORY REQUIREMENTS: NoPosition RequirementsMINIMUM QUALIFICATIONS:High school diploma AND 5 years of human services experience ORCombination of one or more years of college, plus related work or life experience together equaling four years, ORAssociates degree + 2 years of related work or life experience ORBachelor's degree in human services field from an accredited college or university, rehabilitation or nursing.
