Care Transition Coordinator RN

We are seeking a Care Transition Coordinator RN to join our team!Growing agency, great leadership team, room for growth and work-home life balance!Main Duties Of Position Clinical: perform needs assessments, admissions, maintain an open line of communication (customer connections/clinical team/sales teams/referral source), documentation due within 24hrs Sales duties: maintain and build relationships with hospital and all personnel, providing education to all entities within the hospital and patient families, responsible for strategic planning for the accountWork Schedule: Monday-Friday 8am-4:30pm-one weekend a month of sales on call (making 3 sales call per weekend)Qualificationsv Problem solving skills, flexibility, coachability, communication skills, professionalismv 2 Years Of RN Experience Required.v Prior hospice experience and/or sales healthcare sales experience.
