Murrieta Health and Rehabilitation Center - Candidates with a minimum of 1 year Case Management Experience with an LVN license will be prioritized.POSITION SUMMARYThe purpose of your job position as Case Manager or Care Coordinator Nurse is to provide direct nursing care to residents, including the coordination of care that is safe, timely, effective, efficient, equitable, and resident-centered. You are responsible for updating the resident care plans according to the residents condition in coordination with other members of the nursing team. You are also responsible for the education of residents and families on how to follow the current treatment plan. You will use your direct care nursing expertise to serve as the resident/family advocate and act as liaison between medical professionals, payers and allother stakeholders involved in medical care coordination and execution of the resident's patient care plan, including the Administrator, Director of Nursing and other staff.ESSENTIAL DUTIES AND RESPONSIBILITIESExecute the patient care plan.Utilizing your direct care nursing expertise, contribute to the development of a safe clinical plan of care, timely, effective, efficient, equitable, and resident-centered.Review medical records of each assigned resident daily in order to accurately manage resident's progress in accordance with their patient care plan.Coordinate treatment plans and case management with IDT members.Help residents achieve clinical wellness and autonomy.Help residents and family members make informed decisions with the clinical care team by acting as their advocate regarding their clinical status and treatment options.Build trusting relationships with patients to keep track of their progress.Assess and address motivational and psychosocial issues with residents and serve as resident advocate in collaborate with Social Services as necessary.Utilizing your direct care nursing expertise, record cases information, complete accurately all necessary forms and produce statistical reports.As a nurse and utilizing your direct care nursing expertise, participate in the coordination of care with all payers.Follow up with residents' post-discharge.Communicate by phone or in writing regarding the resident's status and changes in care plan to all relevant parties.Using your direct care nursing expertise, document resident progress and document all payer interactions regarding resident progress and expected outcomes.Report any occupational exposures to blood, body fluids, or other hazardous materials to a supervisor immediately.Protect each residents file and data with confidentially per HIPAA standards.Abide with all facility policies and procedures including not disclosing user ID codes and passwords.Adhere to professional standards and Facility policies as outlined by protocols, rules and regulations.Every effort has been made to identify the essential functions of this position.
