Home Health Licensed Practical Nurse

Home Health LPNAs a Home Health LPN, you will: Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN. Implement individualized care plans in collaboration with the patient, family, and healthcare team. Educate patients and their families on disease management, treatment options, and self-care techniques. Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes. Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes. Participate in care coordination activities and discharge planning.
