About us Heavenly Gates Hospice Inc. is a small business in Simi Valley, CA. Our goal is to provide and individualized program of physical, emotional, spriritual, and practical care for people in the last phases of a life-limiting illness, with an emphasis on control of pain and other symptoms. Hospice personnel respect and respond to the unique differences in family/ caregiver, lifestyle, values, and wishes. Our organization is seeking a Licensed Vocational Nurse to help support our team of experts. Our Licensed Vocational Nurse will be responsible for assisting Registered Nurses in providing care to patients, and will also be involved in helping to maintain the medical aspects of our facilities. The right candidate will have reliable transportation and excellent communication skills. Responsibilities: Work as part of a healthcare team. Communicate clearly with other healthcare workers. Follow protocols for a variety of healthcare procedures. Work with patients and families regarding concerns and care plans. Manages all daily clinical records functions including establishing and implementing clinical records policy Receives case refferals. Reviews available patient information related to case. Reviews and evaluates each case Reviews patient's medical diagnosis, prognosis, medications, prcedures and clinical course Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and developing plan of care Job Type: Full-time Pay: $32.00 - $35.00 per hour Work Location: In person
