This role will focus on evaluating, treating, documenting, and monitoring patients with wounds, pressure injuries, and skin integrity concerns.The ideal candidate is clinically strong, detail-oriented, and passionate about delivering high-quality patient care.Responsibilities:Perform wound and skin assessmentsDevelop and implement individualized treatment plansMonitor healing progress and modify treatments as necessaryCollaborate with physicians and interdisciplinary teamsMaintain accurate EMR documentationEducate staff, patients, and families on wound prevention and careEnsure compliance with infection control and regulatory standardsParticipate in quality improvement initiativesQualifications:Active RN license in New YorkPrevious wound care experience preferredExperience within skilled nursing, rehab, or long-term care strongly preferredWound Care Certification (WCC or WOCN) preferred but not requiredStrong communication and organizational skillsAbility to work independently while collaborating with clinical teamsInterested in hearing more?
