Post Acute Care- Nurse Practitioner

We are seeking a Nurse Practitioner to join our growing team. The ideal candidate will have a background in primary care, internal medicine, geriatrics, or hospital medicine, and a passion for serving the post-acute and long-term care population. Key Responsibilities Deliver primary care services within post-acute and long-term care settings. Conduct physical examinations and order/interpret diagnostic tests. Diagnose and manage acute and chronic conditions. Prescribe medications and develop treatment plans as needed. Educate patients and families on preventative care and chronic disease management. Collaborate with physicians, nursing staff, and interdisciplinary teams to ensure continuity of care. Provide ongoing patient counseling and support for optimal health outcomes.
