TheCase Manageris responsible for the delivery of care coordination services to a population with history of substance use and serves as a member of the interdisciplinary treatment team. In this role, you will: Provide clinical case-management to clients who reside in the residence. Support to enhance clients’ capacity to benefit from treatment regimen. Crisis intervention to assist clients in dealing with sanctions, barriers to employment and self-sufficiency. Engage in community outreach to locate appropriate clients & assist through enrollment process. Assess and support services to facilitate access to specialized services. Assist in facilitating access to job training, job search activities, placement and stabilization. Advocacy and report of client activity to community based organizations and clients’ referral / mandating sources. Adhere to agency protocols for maintenance and security of agency equipment. Participate in weekly case conference. Complete client-centered treatment plans. Meet with each assigned client weekly to identify progress towards case management goals. Provide transportation to required appointments when necessary. Observe medication self-administration under supervision. We require: CASAC-T; CASAC, or higher preferred. Minimum of 1 year of experience working with clients with substance abuse disorders and with adolescents with co-occurring and mental health problems. Minimum of 1 year of experience working in in an OASAS funded facility a plus. Commitment to providing quality services and outcome driven performance measures. Excellent organizational management skills with the ability to coach staff, manage, and develop high-performance clinicians, set and achieve program objectives. Strong networking and public relations skills with the ability to engage a wide range of stakeholders, populations, and cultures.
