Community Senior Care Coordinator/Nurse Practitioner

Job DescriptionAs a Community Senior Care Coordinator/Nurse Practitioner, you will be part of the largest multi-specialty care system in the Twin Cities. This position provides the primary health care for patients in assigned care settings in the Minneapolis/St. Paul metro area. This position provides care coordination to achieve patient centered, high quality and cost effective care across the continuum.Qualifications The position requires certification as a Nurse Practitioner (Adult/Geriatric/Family) by a national certifying body (ANCC, AANP, ect.) A current unrestricted Minnesota APRN license as a Nurse Practitioner Must be able to provide own transportation for local travel. May provide service to patients at multiple clinic locations and/or hospital sites. Preferred Qualifications Minimum of one-year experience working in acute care preferred for those assigned to the transitional care settings. Knowledge of Medicare Guidelines preferred for those assigned to long-term/transitional care settings. 3-5 years’ experience working with seniors in a clinical setting You will be joining a team that is supportive and respectful of one another and deeply committed to the mission of HealthPartners.
