PCA CRMA/CNA/Care Manager

We are seeking an experienced and proactive Field Care Manager to oversee the quality and consistency of care provided by our caregivers. This part-time role is crucial to ensuring our services meet all regulatory standards and client expectations. Responsibilities: Supervisory Visits: Conduct regular on-site supervisory visits to assess and verify that caregivers are delivering services in alignment with the client's care plan, company policies, and state regulatory requirements. Initial Client Assessments: Meet with new clients and their families to perform thorough assessments, identify care needs, and develop individualized care plans to ensure the client’s safety, comfort, and satisfaction. Caregiver Introductions: Facilitate meet-and-greet sessions between caregivers and clients, ensuring that there is a good fit and that both parties are comfortable with the arrangements. Caregiver Recruitment: Conduct in-person interviews with prospective caregivers to evaluate their qualifications, experience, and ability to meet the agency’s high standards for care. Provide input on hiring decisions to ensure the team consists of competent and compassionate staff. Ongoing Client Support: Maintain consistent communication with clients and their families to address any questions, concerns, or issues. Ensure that care plans are regularly updated as the client’s needs evolve, and resolve any service-related problems that may arise. Compliance and Documentation: Accurately document all visits, assessments, and client interactions, ensuring compliance with state regulations and agency policies. Ensure that all reports and documentation are completed in a timely and professional manner. Caregiver Support: Act as a resource for caregivers, providing guidance, support, and feedback to help them deliver exceptional care. Address any performance issues or additional training needs to ensure high standards are maintained. Qualifications: Experience: 3-5 years of supervisory or client care management experience in home health care, personal care services, or a related healthcare field. Regulatory Knowledge: Strong understanding of Maine’s healthcare regulations, including compliance standards for personal care agencies and Medicaid requirements. Communication Skills: Excellent communication and interpersonal skills, with the ability to establish trust and rapport with both clients and caregivers. Leadership Skills: Demonstrated ability to supervise and support caregivers in the field, providing clear guidance and ensuring adherence to client care plans. Organizational Skills: Exceptional organizational and time-management skills, with a keen eye for detail in documentation and follow-up. Self-Motivation: Ability to work independently, manage a flexible schedule, and prioritize multiple tasks in a field-based role. Travel: A valid driver’s license and reliable transportation to conduct supervisory visits and client assessments across Maine. Benefits: Competitive Pay: Pay is increased with experience.
