Job Title: Physical Therapist (PT) – Home HealthLocation: Salem, Eugene & McMinnville, ORPosition Type: Full-timeSalary: $100,000 (Benefits + Sign-on Bonus)Role Summary:Client is seeking a licensed Physical Therapist (PT) to join a leading home health provider, delivering high-quality, one-on-one care to patients in their homes across Salem, McMinnville, and Eugene, OR. This role focuses on improving patient mobility, independence, and overall quality of life through personalized treatment plans and collaborative care.Key ResponsibilitiesConduct comprehensive in-home patient evaluations and develop individualized treatment plansProvide therapeutic exercises, manual therapy, and functional mobility trainingEducate patients and caregivers on safety, home exercise programs, and long-term care strategiesCollaborate with an interdisciplinary team including nurses, occupational therapists, social workers, and physiciansAccurately document patient progress and treatment plans in EMR systemsEnsure compliance with HIPAA, safety, and clinical standardsTravel between patient homes; mileage reimbursement providedQualificationsActive Oregon Physical Therapist license (Master’s or Doctorate preferred)Open to new graduates; home health experience is a plusCurrent CPR/BLS certificationValid driver’s license with insured, reliable transportationStrong clinical, communication, and patient education skillsAbility to work independently and manage a dynamic caseloadBenefits:Retirement plan (401k)Health, vision, and dental insuranceCompany-paid life insurancePaid time off (holidays, sick leave, vacation)Sign-on bonus: $5,000–$20,000 (based on experience)
