Licensed Practical Nurse Charge Nurse

The Licensed Practical Nurse (LPN) provides direct nursing care and clinical support to residents within Lenbrook's continuum of care community. This role is responsible for delivering high-quality, compassionate nursing services while supporting the interdisciplinary care team to ensure residents maintain the highest possible level of health, independence, dignity, and well-being.The LPN works under the supervision of a Registered Nurse (RN) or Director of Nursing and plays a key role in resident care coordination, medication management, clinical monitoring, and communication with residents, families, and healthcare providers.Key ResponsibilitiesClinical CareProvide direct nursing care to residents in accordance with physician orders, nursing standards, and regulatory requirements.Administer medications and treatments safely and accurately following physician orders and community protocols.Monitor residents' health status and report changes in condition to the RN, physician, or appropriate clinical leadership.Perform routine nursing procedures including wound care, vital signs monitoring, injections, catheter care, and other treatments as permitted by scope of practice.Assist in the development, implementation, and updating of individualized resident care plans. Resident Monitoring & DocumentationMaintain accurate and timely clinical documentation in the electronic health record system.Observe residents for changes in physical, emotional, or behavioral condition and communicate updates promptly.Participate in resident assessments and interdisciplinary care planning meetings. Care CoordinationCollaborate with physicians, therapists, caregivers, and other healthcare professionals to ensure coordinated resident care.Communicate effectively with residents and families regarding care needs, health concerns, and treatment plans.Provide guidance and support to Certified Nursing Assistants (CNAs) and care staff. Medication ManagementAdminister and document medications in accordance with state regulations and Lenbrook policies.Monitor residents for medication effectiveness and potential side effects.Ensure proper storage, handling, and documentation of medications. Compliance & SafetyAdhere to all federal, state, and local regulations governing long-term care and senior living communities.Follow infection control procedures and safety protocols.Participate in quality improvement and risk management initiatives. Resident ExperiencePromote a resident-centered approach that supports dignity, independence, and quality of life.Build trusting relationships with residents and their families.Support a positive and collaborative care environment.EducationGraduate of an accredited Licensed Practical Nursing (LPN) program. LicensureCurrent Licensed Practical Nurse (LPN) license in the State of Georgia.Current CPR/BLS certification. ExperienceMinimum 1-2 years nursing experience preferred.Experience in senior living, assisted living, skilled nursing, or long-term care strongly preferred. Knowledge, Skills, and AbilitiesStrong clinical assessment and nursing skills.Knowledge of geriatric care and age-related health conditions.Excellent communication and interpersonal skills.Ability to respond calmly and effectively in emergency situations.Proficiency with electronic health records and documentation systems.Strong commitment to resident service and compassionate care. Physical RequirementsAbility to stand, walk, bend, and lift as required in a healthcare environment.Ability to assist residents with mobility and transfers when necessary.Ability to work flexible schedules including evenings, weekends, and holidays as needed.
