DescriptionPOSITION: Licensed Professional Counselor/Social WorkerDEPARTMENT: Medical Services STATUS: ExemptREPORTS TO: Director of Clinical CarePosition SummaryServes as a primary care Behavioral Health Consultant team member providing consultation for patients and their primary care providers within the collaborative care modelDuties And ResponsibilitiesDevelops, implements, reviews, and regularly improves Clinical Treatment Plans to ensure compliance with all relevant regulations and best practicesDischarge planning & manages facility relationships with aftercare providers to ensure each patient is set up for continued success in his or her recovery journeyBiopsychosocial Evaluations and Individualized Treatment Plans; understands chemical dependency and Participates in Treatment Team meetings to evaluate progress of all patients and make recommendations for adjustments to Treatment PlanConducts individual patient therapy sessions, provides brief, same-day consultations and crisis intervention as neededProvides direct support to clinic medical staffConducts trauma informed care and Human Trafficking careUtilizes behavioral health screening tools as part of patient care (e.g., PHQ-9, GAD-7, PSC-17, Social Determinants of Health)Conduct CANS Assessment; conducts other psychosocial assessments, as neededRecords and maintains progress notes for all encountersConducts appropriate discharge of patients and makes appropriate referrals for patients, including psychiatricProvides traditional individual, family or group psychotherapy services, as needed Performs case management duties as necessaryParticipates in case staffing and administrative meetings with clinical supervision and seeks peer consultation as needed Ensures delivery of medical care in accordance with the Catholic Moral and Ethical Directives Follows all HIPAA, OSHA, and licensing guidelines, regulations and code of ethics Maintains confidentiality of patient information/records at all times Maintains established San José Clinic policies, procedures, objectives, quality assurance, safety, environmental and infection control Implements job responsibilities in a manner that is consistent with the San José Clinic Mission and Code of Conduct and is supportive of San José Clinic cultural diversity objectivesRequirementsQualifications, Education, and Experience:LPC/LCSW – licensed in the State of Texas and functions within the Code of EthicsMaster’s degree in counseling psychology, counseling, Social Work or equivalent field, preferredMinimum 5 years’ experience in a clinical role within Chemical Dependency or Human Trafficking field with a minimum of 2 years managerial experienceAt least 2 - 4 years of experience in the medical field or within an integrated primary care setting, working with behavioral health issues related to chronic disease management, depression, anxiety and trauma Must be committed to drug-free lifestyle and responsible alcohol useMust have previous counseling experience with strong clinical skills with adults, children, and adolescents and knowledge and experience in family systemsMust have strong clinical and assessment skills, cultural competence and basic casework skills with referrals and advocacyDemonstrates excellent organizational skills, multi-tasking and effective use of timeAble to handle and complete multiple tasks or projects with multiple deadlinesAble to deal professionally, courteously and efficiently with public and all levels of the organizationAble to operate computers, scanners, and copier faxesMust be proficient in Microsoft Office and practice management system software applicationDemonstrates effective verbal and written communication skillsDemonstrates effective customer relation skills, working with diverse populations, often in stressful fast paced environments Bilingual in English/Spanish, requiredPhysical RequirementsAbility to sit, stand, bend and stoop for (long) periods of timeAbility to exert up to 50 pounds of force occasionally/ frequentlyAbility to respond to emergency/crisis situationsExposure to noiseExposure to blood and/or fluids
