This role involves managing a full caseload, including admissions, ongoing visits, and coordination of care across a diverse patient population. The ideal candidate is highly organized, independent, and experienced in OASIS documentation and home health workflows.Start: 5/25 | 13 weeks8-hour days shiftRecent Home Health experience (within 2 years)Strong OASIS documentation skillsKey ResponsibilitiesPerform Start of Care (SOC), recertifications, and routine visitsComplete OASIS documentation accurately and timelyManage full episode of care for assigned patientsProvide skilled nursing care including:- Wound care- IV therapy and port management- Lab draws- Chronic disease managementCoordinate care with physicians and interdisciplinary teamMaintain timely and accurate EMR documentation (Epic preferred)Participate in on-call and weekend rotationsPatient Load & Travel- Average 5–6 patient visits per day- Travel may include up to 150 miles in a day (not daily)If you're a compassionate RN ready for your next adventure, let's connect!Apply today!
