This role follows a 7 on / 7 off-style rotation, delivering consistent, supportive care to patients and their families while working collaboratively with the interdisciplinary team. The ideal candidate demonstrates flexibility, strong clinical skills, and a commitment to comfort-focused care.Schedule & Work Details:Location: Greater Lake Charles areaSchedule: 7 on / 7 off rotation (averaging 6 full working weeks over a 13-week assignment)Monday through Friday: 11:00 AM to 8:00 PMSaturday and Sunday: 9:00 AM to 9:00 PMWeekly Guarantee: 40 hoursOn-Call/Weekend Requirement: Yes, included in rotationMileage Reimbursement: Yes, between patient visits (first to last patient)Pay Rate: $51 to $53 per hourKey Responsibilities:Provide skilled nursing care to hospice patients in their homes or care settingsPerform comprehensive patient assessments and monitor changes in conditionDevelop, implement, and update individualized plans of care in collaboration with the interdisciplinary teamProvide symptom and pain management to ensure patient comfort and dignityDeliver emotional support and education to patients and families regarding disease progression and end-of-life careCoordinate care with physicians, social workers, chaplains, and other team membersMaintain accurate, timely, and compliant documentation using EMR systemsParticipate in on-call rotation as scheduled and respond to patient needs as requiredRequired Qualifications:Active Registered Nurse (RN) licensePrior hospice nursing experience requiredStrong clinical assessment and critical thinking skillsAbility to work independently in a field-based settingPreferred Qualifications:Experience with Homecare Homebase (HCHB)Patient-centered approach with a compassionate bedside mannerPositive, flexible, and team-oriented attitudeStrong communication and organizational skillsAbout PRIDE HealthPRIDE Health is the minority-owned healthcare recruitment division of Pride Global—an integrated human capital solutions and advisory firm.
