(2) Re-Entry Clinician

Position Specific:Develops and coordinates clinical services to consumers at the County Justice Facility including: Individual, group and family intervention. Community outreach and crisis interventions Referral, advocacy for linkage to essential services. Linkage of consumer to social, spiritual and other community resources as expressed by consumer's wishes and established service plan/discharge goals. Provides eligibility assessments, clinical assessments resulting in care/discharge planning, diagnoses utilizing the Diagnostic and Statistical Manual of Mental Disorders Volume 5 (DSM-V), Positive Behavioral Supports, Motivational Interviewing, and crisis intervention. Participates as member of interdisciplinary team including but not limited to County Justice Facility, the Court systems and company programs. Interprets and ensures self are in compliance. Abides by Agency/NASW Code of Ethics. Title SpecificAssist consumers with transitioning into the community including consumers experiencing homelessness, mental health and substance use disorders. Provide Positive Behavioral Supports, Motivational Interviewing, and crisis intervention (based on the consumers' plan of care and ongoing assessments). Develops and maintains consistent positive collateral, justice involved and community relationships. Prepares documentation of work utilizing agency information systems consistent with agency/program expectations regarding timeliness and content for billing purposes and compliance. Participates in the tracking of individuals who display high rate of recidivism. Executes appropriate case finding / allocation to ensure contract commitments are met/exceeded.
