Utilizing standardized assessments and evidence-based practices, the therapist will implement clinical interventions to aid patients in their recovery.Responsibilities Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care. Assess patient progress and adjust treatments accordingly, documenting all regulatory and clinically required information electronically. Evaluate treatment outcomes for effectiveness and modify services based on patient evaluations. Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders. Communicate effectively with supervisors and health team members about patient progress and barriers to treatment. Write comprehensive discharge summaries of services provided in accordance with regulatory and clinical requirements. Document treatment minutes accurately per all regulatory and clinical requirements. Participate in required meetings, including patient care conferences and rehabilitation meetings. Supervise and support physical therapist assistants, rehabilitation aides, and/or students as needed.Required Experience / Certifications / Licensure Minimum of 1 year of Physical Therapist experience required; new graduates considered if they have Level II experience. Experience in long-term care or rehabilitation settings is required.
