We’re looking for a compassionate, resourceful Case Manager to support a senior housing community in Buellton, CA. This is a hands-on, on-site role where your work directly impacts people’s ability to remain safely and successfully housed. What You’ll Be DoingDeliver individualized, strengths‑based case management grounded in each resident’s unique needsConnect residents to critical community resources (mental health care, financial assistance, senior services, veteran supports, and more)Partner closely with outside caseworkers, agencies, and community organizations to build strong support networks Support residents as they successfully transition into housing after homelessnessHelp individuals navigate challenges related to health, housing stability, and organization/hoarding concerns with compassion and respect Foster connection and belonging by supporting on‑site community activities like meetings, groups, gardens, classes, and wellness eventsWhat We’re Looking ForBachelor’s degree (required) or Master’s degree (MSW, MFT, or related, preferred)A minimum of 3 years of case management or related social services experience (depending on education level)Experience working with seniors, formerly unhoused individuals, or high-acuity populations is strongly preferredComfortable working independently while collaborating with external partnersStrong communication, organization, and follow-through skillsSpanish speaking is a plus, but not requiredMonday–Friday, 8:30 AM–5:30 PM | 📍100% on-site in Buellton, CA | 💰62.4K-72.8K/yr.
