Patient Flow Assistant

Responsibilities:Assists the Bed Census Management Nurse and Nursing Supervisor with coordinating patient transfers from outside facilities, ensuring appropriate placement based on level of care.Facilitates physician‐to‐physician communication throughout the patient transfer process.Collaborates with interdisciplinary healthcare team members to coordinate efficient and timely patient transfers.Monitors transfer timelines, maintaining awareness of delays and providing timely status updates to all stakeholders.Serves as a liaison between UHS Hospitals and referring/sending facilities to ensure clear and consistent communication.Provides high‐quality customer service via phone, electronic communication, and in‐person interactions.Partners with Physicians, Nursing Supervisors, and external facilities to accurately complete Transfer Center documentation.Collects, enters, and maintains Transfer Center data to support reporting and operational tracking.Adheres to established organizational policies, procedures, and regulatory standards.Performs other duties as assigned in support of departmental and organizational needs.Education/ExperienceMinimum Required:High School Diploma, 1 year experience working in a healthcare related field.Preferred:Associates DegreeLicense/CertificationPreferred:EMT, MOAWork EnvironmentWork is performed in an office environmentAge of Patients ServedAll Age GroupsHIPAA Roles‐Based Access to Patient InformationClinical ‐ Access to patient information that is not restricted (an example of restricted information is ARU) ‐ Level 3
