ADRC/Aging Unit - Pool Registered Dietitian
This position provides individualized nutrition counseling for clients with high nutrition risk or who have requested nutrition counseling. This position also supports and promotes the Mission, Vision and Guiding Principles of the Human Services Department and performs in a manner consistent with the HSD Professional Standards and "Commitment to My Co-Workers."
80% - Nutrition Counseling:
Determine clients desired outcome of Nutrition Counseling
Complete General Services Registration form
Discussion of possible diet changes to address concerns
Present client with recommended plan to address nutrition concerns
Provide client with Nutrition Counseling summary
Provide follow-up with client if requested
Enter data into Peer Place
15% - Preparation for Participant
Research client-specific health conditions/needs
Develop a tentative plan to address nutrition concerns
5% - Performs other duties as assigned. (Potentially assist with Classes, Presentations or Sanitation visits as needed.)
Advanced knowledge of frailties of aging.
Experience and skill in researching specific medical conditions as they relate to nutrition.
Experience in developing collaborative relationships, preferably with community organizations and health care professionals.
Ability/competence to engage clients in discussions.
Ability to provide culturally competent services and supports.
Ability to function independently and be resourceful in the roles undertaken.
Computer skills including data collection and reporting.
Ability to be available / flexible to meet with clients for Nutrition Counseling.
Bachelors degree from an accredited school with major in Nutrition, preferred.
Wisconsin Certified.
Registered Dietitian with the Academy of Nutrition and Dietetics/ Commission on Dietetic Registration.
Minimum of 5 years experience working directly with older adults, preferred.
A valid Wisconsin Drivers License and ability to travel community events/homes.
Physical Elements

Indoor--often
Outdoor--rarely; primarily as a result of travel requirements to & from the office, client homes, schools, and other meeting sites in the community
